Memorial Board Plaque & Simcha Tree of Life Leaf Orders

You may use this form to order and pay for a Memorial Board plaque and/or Simcha Tree of Life leaf.


A Memorial Board plaque is created in memory of a loved one and will be mounted in our Meditation Room near the Sanctuary.   
 
A Simcha Tree of Life leaf adorns a decorative tree near our Sanctuary and may be dedicated in honor of any simcha.  Examples would be Mazel Tovs for B'nei Mitzvah, Confirmation Class graduations, and special birthdays and anniversaries.
