Beth El Covid Waiver

Beth El is committed to member, visitor, and staff safety and health during this troubled period. As the building and grounds reopen slowly, we ask for your participation in supporting this commitment.   

Because your status and ours may change over time, we ask that you submit a waiver for each event you plan to attend. Waivers are only required if you plan to or may need to enter the building. Please itemize below the family/group members who will attend with you and to whom this waiver applies, including yourself if you are attending.

Attention Members: You do not have to log in to Shul Cloud to fill in this waiver form, but if you do, your identifying information below will auto-fill for you and save you time.
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. As a result, federal, state, and local governments and agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.We are doing everything we can to be compliant with all regulations and ensure your safety. We have put in place preventative measures to reduce the spread of COVID-19, but we cannot guarantee that you or family members will not become infected with COVID-19.

By participating in programs, services, and activities of Beth El Hebrew Organization, you agree to the following:

On behalf of yourself, your minor children, and others named below, you acknowledge that every  participant is aware of the transmission of COVID by person-to-person contact, and by touching surfaces touched by others, which transmission is enhanced by events where people congregate and do not maintain social distancing and do not adhere to mask-wearing, and that you are willingly assuming a known risk of being infected with COVID when you attend events of this nature, whether indoors or outdoors.

On behalf of yourself, your minor children, and others named below, you hereby release, covenant not to sue, discharge, and hold harmless Beth El Hebrew Organization, its employees, agents, and representatives, of and from all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating to your participation in our programs, services, or activities. You understand and agree that this release includes any claims based on the actions, omissions, or negligence of this organization, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any such program, service or activity.
